Marc Egort Delivers as Fractional CFO
As described by Marc Egort, a chief financial officer (CFO) is someone hired to assist the executive team in making decisions that impact them financially. Their job is not necessarily to do the financials, but more to interpret and help use the information as a planning tool. But what if a company isn’t ready to make a fulltime investment in a CFO? That’s where Egort can step in – as a fractional CFO.
Egort explained, “We hire ourselves out to provide fractional CFO services to companies that may not need the services fulltime.” He said reasons for that decision could include that the company would be required to give a fulltime financial officer not just sizable compensation, but also benefits; stock options could be involved as well, and a company may not want to or may not be in a position to do that. That’s when his firm can step in.
“We make ourselves available to provide some pretty specific components, so once financial statements are prepared, we will go in and do some analytics.” He said, for example, they can provide information on the relationship to the client’s budget, in terms of how they performed, or are able to provide information on a cash flow projection – whether there might be a potential deficit over the next six months, so the company can prepare for that.
Egort founded his own public accounting firm, Marc Egort CPA, P.A., in 1999, after having been in public accounting since 1991. His background has been in home health and healthcare since 1989. He said that in going from being a healthcare consultant to financial reporting and tax compliance – his work over the years has evolved into a tax practice, and tax mitigation is one element of it. He has considerable background in cost finding, financial reporting, and analyzing balance sheets, and understanding what moving parts are for all types of businesses. “Our job is then to help educate the business owners along the way.”

Mindy Shikiar was a Vice President at Boca Raton Regional Hospital when she was first introduced to Caldwell Butler (CB) and encountered the firm’s structured approach to operational performance improvement. By the end of the cycle, each leader will have implemented approximately eight targeted improvements.
According to Shikiar, these improvements can generate substantial financial impact, frequently 3 percent to 5 percent improvement in 12 months of their total operating expenses – a meaningful achievement in an industry where margins are often razor-thin. The model also helps cultivate a culture of continuous improvement.

Cleveland Clinic Joins 22 Health’s Marketplace Network to Provide Greater Access to Patient Care in Broward County, Fla.
22 Health Members Now Have Access to Approximately 12,000 Caregivers at Cleveland Clinic’s Hospitals and Outpatient Centers
Nov. 26, 2025 – Cleveland Clinic has announced that it is joining the provider network of 22 Health, a new health marketplace division of Community Care Plan serving residents of Broward County, effective Jan. 1, 2026. Under the terms of the new agreement, 22 Health members will have access to approximately 12,000 additional caregivers across the five Cleveland Clinic hospitals and 40 outpatient centers throughout Florida.

Sylvester Comprehensive Cancer Center Pairs Scientific and Clinical Excellence to Improve the Lives of Pediatric Cancer Patients
When Daniel Armstrong, Ph.D., professor of pediatrics and director of the Mailman Center for Child Development at the University of Miami Miller School of Medicine, arrived in Miami in 1985, the survival rate for children with cancer was about 50% in the first year after diagnosis.
Dr. Armstrong got to work collaborating with other researchers and clinicians at Sylvester Comprehensive Cancer Center, part of the Miller School. He studied how cancer treatments affected patients’ developing brains, and he witnessed the growth of clinical trials to advance treatments.
Soon, he noticed his hospital rounds were taking longer.
“We were still seeing new patients, but my caseload was climbing exponentially because children were surviving,” Dr. Armstrong said.
Today, the survival rate is about 85% for all pediatric cancers, and many types are more than 90%, said Dr. Armstrong, now chair of the Sylvester Children’s Cancer Initiative, which is working to build on the cancer center’s strengths.

Mount Sinai Medical Center reaches topping out milestone for new Westchester Emergency Center
During topping out, crews often sign the final steel beam to be placed in the project. In addition to the traditional beam, the Westchester Emergency Center construction crew signed a 245,000-pound concrete wall to recognize the tilt-up construction project’s complexity.
Nov. 25, 2025 — Construction crews have placed the final beam atop Mount Sinai Medical Center’s new Westchester Freestanding Emergency Department, marking a major milestone in expanding access to high-quality, around-the-clock care for residents in western Miami-Dade County. The new facility will bring Mount Sinai’s trusted emergency and specialty care closer to home for thousands of area families, helping reduce travel times and meet growing healthcare needs.
Located in Westchester, the four-story, 112,000 square-foot facility—designed by HuntonBrady Architects—will feature 26 exam bays, a Baker Act receiving unit, and advanced imaging services such as MRI, CT, and ultrasound. The project also features a detached two-story central energy plant and medical gas building, demolition of two existing structures and six acres of site improvements. Construction is expected to be complete late 2026.
“This milestone is a tribute to the skill and dedication of every crew member who’s helped bring this facility to life,” said Mike Peters, vice president and division manager at Brasfield & Gorrie. “Partnering with Mount Sinai Medical Center and HuntonBrady Architects on this important project highlights what can be achieved through teamwork and craftsmanship. Sarasota Memorial’s Brian D. Jellison Cancer Institute Named Center of Excellence for Lung Cancer Detection & Care
November 20, 2025 — Lung cancer is the leading cause of cancer death in the United States, in large part because nearly half of all cases aren’t detected until late in the disease, when survival rates fall to just 6%. But early detection screening programs and coordinated care provided at Sarasota Memorial’s Brian D. Jellison Cancer Institute are helping people beat those odds.
The local Cancer Institute team has been nationally recognized for catching early-stage lung cancer at double the rate achieved in most health systems across the nation. And this month, during Lung Cancer Awareness Month, the nonprofit GO2 for Lung Cancer awarded SMH two Centers of Excellence designations for providing multidisciplinary, top-tier care across the full continuum of cancer care services:
-Center of Excellence for Cancer Care, for providing comprehensive access to a continuum of oncology care, including medical, radiation, and surgical oncology and pathology services.
-Center of Excellence for Incidental Pulmonary Nodules, for providing a structured program that captures incidentally found lung nodules from imaging performed across the health system, with a standardized process for patient follow-up and support.
The newly awarded Center of Excellence designations are in addition to the Cancer Institute’s previously awarded Center of Excellence designation for Lung Cancer Screening, which recognizes the life-saving impact of early detection.

HCA Florida Ocala Hospital and HCA Florida West Marion Hospital are celebrating a successful first year of a three-year grant program by congratulating hundreds of local high school students for learning more about healthcare, including pursuing potential careers in the field.
HCA Healthcare Foundation, through its Healthier Tomorrow Fund, announced a $1 million grant last year to the Consortium of Florida Education Foundations to help fund its Career Pathways to a Healthier Florida program. The Public Education Foundation of Marion County (PEFMC) received $60,000 from the grant to support students in Belleview High School, Lake Weir High School, North Marion High School, and West Port High School.

ASTP/ONC to exercise enforcement discretion for health IT developers for certain certification criteria
NOVEMBER 26, 2025 – The Assistant Secretary for Technology Policy/Office of the National Coordinator for Health IT announced Nov. 24 that it will exercise temporary enforcement discretion for health IT developers regarding certification criteria compliance dates and attestations of condition and maintenance for certification to account for the impact of the lapse in government appropriations that rendered the ASTP/ONC website unavailable, including access to attestation submission portals, testing tools and other resources.
ASTP/ONC will exercise temporary enforcement discretion for compliance with certification program criteria dates for certain standards, effective Jan. 1, 2026, through March 1, 2026. In effect, health IT developers will have until Feb. 28, 2026 to complete updates to health IT modules for those criteria required under the Health Data, Technology, and Interoperability: Certification Program Updates, Algorithm Transparency, and Information Sharing (HTI-1) final rule.

Mayo Clinic researchers identify why some lung tumors respond well to immunotherapy
November 26, 2025 — For some patients with the most common type of lung cancer, known as lung adenocarcinoma, there’s new hope. In a new study published in Cell Reports, Mayo Clinic researchers have found several previously unknown genetic and cellular processes that occur in lung adenocarcinoma tumors that respond well to immunotherapy.
A recently approved group of drugs — immune checkpoint inhibitors — can boost the body’s ability to eliminate a tumor and even keep the cancer from coming back. However, while the medications work well for some people, the drugs aren’t effective for many other patients with the disease — and researchers are trying to determine why.
“Our study describes the events that occur when a patient’s tumor harbors only one copy of a cancer-causing gene, which occurs in 20% of cases,” says Alan P. Fields, Ph.D., a cancer biologist at Mayo Clinic Comprehensive Cancer Center and the principal investigator of the study.
The research team found that the missing tumor-driving gene, known as PRKCI, results in tumors that are less aggressive. The missing gene also cultivates a more powerful immune response to tumors. Surprisingly, the research team found the improved immune response occurs with help from unexpected players: senescent tumor cells, also known as “zombie cells,” which are typically associated with negative consequences of disease and aging.
The study identified markers that may predict a positive response to immunotherapy and “ultimately may help clinicians stratify patients who are candidates for immune checkpoint inhibitors,” says Joey Nguyen, a graduate student at Mayo Clinic Graduate School of Biomedical Sciences and lead author of the publication.

Brian Christine, MD, Assistant Secretary for Health
Brian Christine, MD, is an internationally recognized leader in men’s health. His medical practice is a renowned global Center of Excellence. In addition, Dr. Christine has published peer-reviewed research, lectured widely, and trained surgeons internationally. He is a member of the American Urologic Association, Sexual Medicine Society of North America, the International Society for Sexual Medicine, and the International Continence Society. Dr. Christine earned his M.D. from Emory University and completed his residency in urology at the University of Alabama at Birmingham. On October 7, 2025, the U.S. Senate confirmed him as Assistant Secretary.

CMS Delivers Savings for Seniors on 15 Major Drugs for Cancer and Chronic Disease
NOVEMBER 25, 2025 – The Centers for Medicare & Medicaid Services (CMS) today announced a net savings of 44%, or $12 billion, from last year’s negotiated Medicare spending on 15 drugs used widely to treat cancer and other serious chronic conditions. These savings are the result of the Trump Administration’s aggressive push for lower prices for Medicare beneficiaries.
The Maximum Fair Prices (MFPs) for these 15 drugs will become effective January 1, 2027, bringing the total number of negotiated drugs to 25 when combined with the 10 previously negotiated drugs with MFPs taking effect January 1, 2026. The 15 drugs in this second negotiation cycle, used to treat cancer, diabetes, asthma, and other chronic illnesses, represent some of the highest Medicare Part D spending. The MFPs offer substantial savings for both beneficiaries and the Medicare program.
“President Trump directed us to stop at nothing to lower health care costs for the American people,” said Health and Human Services Secretary Robert F. Kennedy, Jr. “As we work to Make America Healthy Again, we will use every tool at our disposal to deliver affordable health care to seniors.”
“This year’s results stand in stark contrast to last year’s,” said CMS Administrator Mehmet Oz, MD. “Using the same process with a bolder direction, we have achieved substantially better outcomes for taxpayers and seniors in the Medicare Part D program — not the modest or even counterproductive ‘deals’ we saw before.”
“Whether through the Inflation Reduction Act or President Trump’s Most Favored Nation policy, this is what serious, fair, and disciplined negotiation looks like,” said CMS Deputy Administrator and Medicare Director Chris Klomp. “I’m deeply proud of our team, who execute exceptionally well to bring affordability to the country in everything we do.”
Today’s announcement demonstrates the Administration’s continued commitment to lowering prescription drug costs and ensuring Medicare beneficiaries have access to necessary medications at lower agreed-upon prices. By establishing better prices for high-cost drugs, CMS ensures patients experience financial relief at the pharmacy counter.

Calendar Year (CY) 2026 Home Health Prospective Payment System Final Rule (CMS-1828-F)
On November 28, 2025, the Centers for Medicare & Medicaid Services (CMS) issued a final rule that announces policy changes under the Home Health (HH) Prospective Payment System (PPS), consistent with the legal requirements to update Medicare payment policies for home health agencies (HHAs) annually. This fact sheet discusses the major provisions of the final rule.
CY 2026 Payment and Policy Updates for Home Health Agencies
This rule finalizes routine, statutorily required updates to the HH payment rates for CY 2026. The CY 2026 updated rates include the final CY 2026 HH payment update of an estimated 2.4% increase ($405 million increase), which is offset by an estimated 0.9% decrease that reflects the final permanent adjustment ($150 million decrease), an estimated 2.7% decrease that reflects the final temporary adjustment ($460 million decrease), and an estimated 0.1% decrease that reflects the updated fixed-dollar loss (FDL) ratio for outlier payments ($15 million decrease). CMS estimates that Medicare payments to HHAs in CY 2026 will decrease in the aggregate by an estimated 1.3%, or $220 million, compared to CY 2025, based on the finalized policies.
