MIAMI CANCER INSTITUTE LAUNCHES TRIAL OF AZELIRAGON WITH STEREOTACTIC RADIOSURGERY IN PATIENTS WITH BRAIN METASTASES
January 29, 2024 – Physician-researchers at Miami Cancer Institute, part of Baptist Health South Florida, recently launched a clinical trial that studies the combination of a new medication and stereotactic radiosurgery in patients with brain metastases. The therapy combines azeliragon, a blood brain barrier-permeating antagonist of the receptor for advanced glycation end products (RAGE) that is known to affect inflammation, with radiosurgery. The researchers hope the combination therapy will improve outcomes in patients with brain metastases who often have few treatment options.
“The ultimate aim of the study is to find an innovative new approach to reducing tumors, preventing brain inflammation that can follow radiation therapy, and potentially protecting neurocognitive function,” said Rupesh Kotecha, M.D., chief of radiosurgery, director of the Central Nervous System Metastasis Program at Miami Cancer Institute, and principal investigator of the trial. “This is an exciting new combinatorial regimen as it has the potential to overcome cancer’s resistance pathways and improve response rates.”

NEW AI TECHNIQUE SIGNIFICANTLY BOOSTS MEDICARE FRAUD DETECTION
FAU Engineering Study Explores ‘Vast Sea’ of Big Medicare Data, Parts B and D
January 31, 2024 – Medicare is sporadically compromised by fraudulent insurance claims. These illicit activities often go undetected, allowing full-time criminals and unscrupulous health providers to exploit weaknesses in the system. Last year, the estimated annual fraud topped $100 billion according to the National Health Care Anti-Fraud Association, but it is likely much higher.
Traditionally, to detect Medicare fraud, a limited number of auditors, or investigators, are responsible for manually inspecting thousands of claims, but only have enough time to look for very specific patterns indicating suspicious behaviors. Moreover, there are not enough investigators to keep up with the various Medicare fraud schemes.
Utilizing big data, such as from patient records and provider payments, often is considered the best way to produce effective machine learning models to detect fraud. However, in the domain of Medicare insurance fraud detection, handling imbalanced big data and high dimensionality – data in which the number of features is staggeringly high so that calculations become extremely difficult – remains a significant challenge.

BIDEN-HARRIS ADMINISTRATION ANNOUNCES ACTION TO INCREASE ACCESS TO SICKLE CELL DISEASE TREATMENTS
January 30, 2024 – The Biden-Harris Administration announced today that sickle cell disease (SCD) will be the first focus of the Cell and Gene Therapy (CGT) Access Model, which was initially announced in February 2023. The model is designed to improve health outcomes, increase access to cell and gene therapies, and lower health care costs for some of the nation’s most vulnerable populations.
Sickle cell disease is an extremely painful condition, which disproportionately impacts Black Americans and has had limited treatment options. In the United States, more than 100,000 people live with SCD. Individuals with the disease have a shorter life expectancy, by more than 20 years, compared to someone living without SCD. Additionally, many long-term health complications from SCD — including stroke, acute chest syndrome, and chronic end-organ damage — can lead to higher rates of emergency department visits and hospitalizations. Patients with SCD experience challenges with access to quality and affordability of care. This model has the potential to help improve health outcomes for patients and families with SCD while also ensuring taxpayer dollars are being used more effectively.

ABNORMAL PROTEINS FOUND IN THE SPINAL FLUID OF PEOPLE WITH ALS AND FRONTOTEMPORAL DEMENTIA
Proteins could serve as biomarkers that improve diagnosis and guide the development of novel therapies.
January 31, 2024 – Researchers at the National Institutes of Health detected abnormal proteins in the spinal fluid of people with amyotrophic lateral sclerosis (ALS) and frontotemporal dementia (FTD), which could help improve diagnosis of these diseases. The findings were published in Science Translational Medicine.
The proteins in question are built from “cryptic” exons—abnormal portions of RNA, the cell’s instructions for how to build proteins. Cryptic exons occur when TDP-43, a protein that regulates how RNA is processed, stops functioning normally. TDP-43 dysfunction is linked to ALS, FTD, Alzheimer’s disease, and Limbic Associated TDP-43 Encephalopathy (LATE).

NIH STUDY SHOWS HIGHER MORTALITY RATES FOR PATIENTS ON RESPIRATORY SUPPORT IN RURAL INTERMEDIATE CARE UNITS
Findings highlight the importance of providing ICU-level care to rural patients with respiratory failure
January 31, 2024 – A new National Institutes of Health-supported study finds that patients receiving ventilator life support in the intermediate care units – a potentially less costly alternative for people not sick enough for the intensive care units (ICUs) but too ill for the general ward – of rural hospitals had significantly higher death rates than patients in the same type of unit at urban hospitals. The study also found that patients with respiratory failure in the ICUs at rural and urban hospitals fare similarly. Published in the journal Annals of the American Thoracic Society, this study highlights the need for more careful evaluations of patients with breathing problems who are assigned to intermediate care units.
“This study has important implications for rural hospitals when determining how to care for their sickest patients on mechanical ventilation, as rural hospitals tend to be smaller and less resourced,” said Gustavo Matute-Bello, M.D., deputy director for the Division of Lung Diseases at the National Heart, Lung, and Blood Institute (NHLBI), part of NIH. “It emphasizes the need to assess whether rural intermediate care units can meet the complex demands of critically ill patients, and the importance of carefully evaluating the processes designed to care for them.”

The study, published in the Journal of the American Medical Association, highlights a surge in the adoption of complementary health approaches for pain management over the same period.
