Preparing for the Avalanche of Uninsured Patients
Survival Strategies Include Bold Initiatives

The recently enacted federal Medicaid policy changes as part of the One Big Beautiful Bill Act (OBBBA) will affect all state healthcare providers and not-for-profit community agencies, including South Florida healthcare stakeholders who serve a large uninsured population. These changes will adversely impact local municipalities, safety-net hospitals, and rural hospitals, which will face increased financial responsibility for providing care to uninsured patients.
The OBBBA sets limits on healthcare taxes imposed by nearly every state on providers or managed care plans. These taxes generate revenue for supplemental Medicaid payments, and mostly benefit hospitals, but may also support other provider types. At stake in Florida is a portion of the $3.4 billion in provider taxes collected. Of this amount, Dade-County hospitals received $662 million; Broward hospitals, $360 million.1
Community hospitals and other providers may cut or stop care for uninsured patients due to limited finances or operational priorities.
With OBBA Medicaid reform taking effect after December 31, 2026, healthcare providers must prepare for increased uninsured patient volume. Financial stability will depend on good strategic planning, political partnerships, and mitigation efforts.

The healthcare-related provisions of the One Big Beautiful Bill Act (OBBBA) affect various segments of the healthcare industry. Rule implementation timelines differ, with certain Medicaid policy changes postponed until the fourth quarter of 2026. This delay allows organizations to make necessary adjustments in response to revised federal funding levels, updated public assistance program eligibility requirements, and modifications to tax regulations.
Summary of Important Policy Shifts
Immediate Policy Changes
Select not-for-profit entities: For one year, the OBBBA prohibits Medicaid reimbursement to entities primarily engaged in family planning, reproductive health, and related medical care that also provide abortion services (except in cases of rape, incest, or danger to the mother’s life).
Effectively, this rule change ends Medicaid funding for even non-abortion healthcare services for entities such as Planned Parenthood and other similar organizations.
Long-term care entities: Long-term care facilities get one OBBBA related win; a ten-year reprieve on the federal staffing mandate specifying nursing homes to maintain a minimum number of certified nursing assistants (CNAs) and 24/7 registered nurse (RN) coverage. Michelle Marsh, CEO of Forma Advisors opines “this provision will still not reduce staffing shortages in nursing homes. Recruiting challenges will persist for several more years.

Tampa General Hospital is First Health System in Tampa Area to Offer Minimally Invasive Procedure for Tremor
Utilizing specialized radiosurgery technology, this procedure may improve outcomes for tremor patients compared with other available therapeutic interventions.
Aug. 19, 2025 – The Neuroscience Institute at Tampa General Hospital (TGH) now offers stereotactic radiosurgery (SRS) thalamotomy, a new and minimally invasive treatment for tremor, or uncontrollable shaking, in select patients with essential tremor (ET) and tremor dominant Parkinson’s disease (TD-PD). The integration of this revolutionary treatment option now allows for a full suite of services available for patients with movement disorders. TGH is the first health system in the West Central Florida region to offer SRS thalamotomy and one of the few in the nation utilizing a recently developed “frameless” technique that avoids traditional cranial fixation pins (head frame), maximizing patient comfort while providing precise treatment. The innovative and specialized technology delivers high-intensity radiation to a small, precise area in the brain that is responsible for tremor generation. The SRS procedure requires no anesthesia, incisions, drilling or head shaving and, because it is an outpatient procedure, patients can go home the same day.
Tremors in ET and TD-PD can affect almost any part of the body, but most commonly occur in the hands, and can happen at any age, but are usually seen in people 40 and older. While the condition isn’t life-threatening, it can be disabling and worsen over time. If tremors become severe, the condition may cause difficulty in performing everyday tasks, such as eating, drinking, writing, shaving, putting on makeup or even speaking.
“This procedure has the potential to improve the lives of many in our region, as tremors are the most common movement disorder, and SRS has been shown to be safe and effective in providing good tremor control and can improve quality of life in patients for whom medications did not work to control tremors,” said Dr. Mohamed Khattab, radiation oncologist, TGH Cancer Institute, and Assistant Professor, Radiation Oncology, and director of research, CNS Radiation Oncology, USF Health Morsani College of Medicine. “SRS can be particularly beneficial for patients with severe, medically refractory essential tremor and Parkinsonian tremor, meaning that their tremors do not typically see improvement with medications or other therapies.”

August 18, 2025 – Miami Cancer Institute, part of Baptist Health South Florida, today announced its presentations at the 67th American Society for Radiation Oncology (ASTRO) Annual Meeting, September 27 – October 1, 2025, at the Moscone Center in San Francisco, California. The presentations focus on emerging advancements and nuanced evaluations in radiation oncology, innovations in proton and MR-guided adaptive radiotherapy, treatment planning and outcomes for gastrointestinal and brain cancers, automated imaging segmentation tools, motion management, and biological modeling of treatment risks.

What Makes the Clubhouse Model Different
The clubhouse model fundamentally reimagines mental health recovery by centering on community, peer support, and meaningful work. Unlike traditional treatment facilities where individuals are patients receiving services, clubhouses welcome “members” who work alongside staff as equals. This approach recognizes that recovery involves rebuilding lives and reconnecting with society.
The model addresses social isolation by having members participate in running the clubhouse itself, taking on responsibilities in food service, administration, maintenance, and outreach. This work-ordered day structure provides purpose and builds confidence while developing practical skills that translate to employment opportunities.
Evidence-Based Benefits
Research consistently demonstrates the clubhouse model’s effectiveness across multiple recovery domains. A 2018 systematic review found solid evidence that clubhouses increase employment rates, reduce hospitalizations, and improve relationships and quality of life for people with serious mental illness. Studies show clubhouse members experience fewer psychiatric readmissions and demonstrate better long-term outcomes compared to those receiving traditional services alone.

Do No Harm: Rethink Treating Diabetes, Hypertension in Frail Older Adults
August 20, 2025 – Effectively managing chronic conditions like diabetes and hypertension can greatly improve quality of life, reduce complications and extend longevity for older adults. However, when treatment becomes overly aggressive, it can do more harm than good. Every day, thousands of vulnerable older adults in the United States are harmed by intensive management of these conditions – resulting in dangerously low blood sugar or blood pressure, emergency visits, hospitalizations, disability or even death. These harms are preventable and often stem from well-intentioned but excessive medical care.
Despite clear guidelines promoting cautious, individualized care – especially for frail older adults and nursing home residents – these complications remain alarmingly common.
Joseph G. Ouslander, M.D., a professor of geriatric medicine at Florida Atlantic University’s Charles E. Schmidt College of Medicine, and his collaborator, have published a paper in the Journal of the American Geriatrics Society calling for urgent change in how health care providers are supported and held accountable. They argue that physicians, nurse practitioners, and physician assistants prescribing diabetes and hypertension medications should be actively encouraged – not merely expected – to avoid overtreatment through carefully designed quality measures.
