When It Comes To Patient Care, Benchmark the Best Part 4

By Jay Juffre

As we continue to look at what the best of the best are doing to drive patient care, we need to see what organizations are doing to better educate patients and their families. There is an old adage, ‘always listen to your doctor’. This is typically very sound advice. But what does the doctor do when, for the two days leading up to the appointment, the patient has been entering their symptoms into WebMD? The answer of leading Healthcare providers is to educate. Leaders in the industry are focusing on patient education and health literacy.

This is more than bedside chats. They are putting their money where their mouths are by investing in programs to empower patients with the knowledge and skills to make informed decisions about their health, manage chronic conditions effectively, and navigate the healthcare system confidently.

An educated patient has less stress, feels more confident in their diagnosis or treatment, and is in a word happier, with their health care provider. For example, I have followed my primary care provider to different health systems because she goes the extra mile to educate me on anything going on when it comes to my health. There are a few best practices we can glean from the best. First of all, start early

Five Tips for Renegotiating Pharmacy Benefits Manager (PBM) Contracts

By Alfredo Cepero

The U.S. Department of Health and Human Services estimates that $603 billion was spent on prescription drugs in 2021 before accounting for rebates. A significant portion of employers’ healthcare costs are attributed to pharmaceuticals, and these expenses continue to climb. As expensive specialty drugs and new gene therapies become more prevalent in the market, companies must do their diligence when choosing a pharmacy benefits manager (PBM) or renegotiating their existing contract.

The complexity, industry jargon, and overall breadth of these agreements might discourage companies from fully exploring their options and seeking the most competitive rates for their business and employees. Fortunately, there are ways that companies can make sure their PBM contracts are appropriate to their business. Here, we explore five tips to help companies navigate the PBM contract renegotiation process.

Know Your Data
Your data may be the most crucial tool when entering into negotiations on your PBM contract. Having a deep understanding of how your drug spend currently is allocated, how your members are using your plan, your formulary adherence, and utilization by drug class is imperative to understanding your greatest cost levers and potential risks.

Now Out of Bankruptcy, Cano Health Focusing on the Future

Now that Cano Health has successfully emerged from bankruptcy, it is well on the way to achieving its ultimate goal—putting the patient first. With a host of new services coming to market and the strength of its team and community, the health system is positioned to move forward in a new and exciting way.

“It’s really uncommon to have the court approve a reorganization plan and to exit bankruptcy on the same day, but that’s what happened on June 28, 2024,” said CEO Mark Kent, FACHE, FACMPE. “I’m very thankful to our advisors, employed physicians, employees, creditors, vendors, and trusted affiliated physician partners for helping us get through this, and I’m proud to say that they can now expect a new Cano Health where our focus is on the patient experience.”

In addition to moving physical and massage therapy in-house, Cano Health has added physical medicine and rehabilitation (PMR) to its services and enhanced pharmacy services so that patients can get their medications in the medical center at the time of their appointments or provided at home. The company has also expanded the Cano at Home program and Cano Cares program so that patients can interact with the health system 24 hours a day, 7 days a week.

“Patients can receive primary care services from a visiting nurse practitioner in their homes as late as 11 p.m., including on weekends,” said Kent, noting that those with emergency issues should still go to the ER or call 911. “We also have nurse practitioners available by phone until the center opens the next morning; we believe these additions will help our patients along their healthcare journey.”

Alcohol Use in Older Adults Doubles Risk of Brain Bleeds from Falls

FAU Study Among First to Examine Link Between Alcohol Use and Severe Head Trauma

August 8, 2024 – Nationally, falls remain the leading cause of both fatal and non-fatal injuries in older adults and are the leading cause of traumatic brain injury. In 2021, falls led to the deaths of 36,500 older adults in the United States and 3,805 older Floridians. While some studies have hypothesized that alcohol use contributed to these outcomes, there are few studies which have examined this issue.

As such, little is known about the association between the frequency of alcohol use and the severity of injuries sustained after a fall in older adults. A study by Florida Atlantic University’s Schmidt College of Medicine and collaborators, is one of the first to examine the relationship between self-reported alcohol use and severe head trauma in this group.

For the study, researchers used data from the Geriatric Head Trauma Short Term Outcomes Project (GREAT STOP), which included a prospective cohort of consecutive patients presenting to the emergency departments (ED) of two level one, university-affiliated trauma centers in Palm Beach County, with annual volumes of 50,000 and 69,000.

JAMA Oncology Review: The Latest Treatment Options for Acute Myeloid Leukemia

Newer options include less toxic front-line therapies and several targeted drugs
AUGUST 8, 2024 – Recently approved drugs are rapidly changing the treatment of patients with acute myeloid leukemia (AML), with newer options including less toxic front-line therapies and several targeted drugs. These new drug combinations and targeted therapies may offer a brighter future – especially among older patients and others who may be more frail.

Physician-scientists at Sylvester Comprehensive Cancer Center at the University of Miami Miller School of Medicine surveyed the field in an Aug. 8, review published in the peer-reviewed journal, Journal of the American Medical Association Oncology (JAMA Oncology).

“It’s been an exciting time in the treatment of AML,” said Mikkael Sekeres, M.D., M.S., Chief of the Division of Hematology at Sylvester, and senior author on the article. “The time was ripe for an update on the biology of AML and the treatment approaches that we’ve adopted since the approval of these drugs,” he added.

About 20,800 people are diagnosed with AML annually in the U.S., with a median age of onset of 69. Only about a third of patients live five years or longer, according to National Cancer Institute data from 2014-2020.

Historically, a substantial fraction of patients were unable to tolerate the first-line, harsh combination of chemotherapy drugs. Many frailer, typically older patients often did not receive any active therapy.

Cleveland Clinic Study Adds to Increasing Evidence that Sugar Substitute Erythritol Raises Cardiovascular Risk

Erythritol is a common artificial sweetener found in baked goods, beverages, gum and candy

August 8, 2024 – New Cleveland Clinic research shows that consuming foods with erythritol, a popular artificial sweetener, increases risk of cardiovascular events such as heart attack and stroke. The findings, from a new intervention study in healthy volunteers, show erythritol made platelets (a type of blood cell) more active, which can raise the risk of blood clots. Sugar (glucose) did not have this effect.

Published in Arteriosclerosis, Thrombosis and Vascular Biology, the research adds to increasing evidence that erythritol may not be as safe as currently classified by food regulatory agencies and should be reevaluated as an ingredient. The study was conducted by a team of Cleveland Clinic researchers as part of a series of investigations on the physiological effects of common sugar substitutes.

“Many professional societies and clinicians routinely recommend that people at high cardiovascular risk – those with obesity, diabetes or metabolic syndrome – consume foods that contain sugar substitutes rather than sugar,” said senior and corresponding author Stanley Hazen, M.D., Ph.D., chair of Cardiovascular and Metabolic Sciences in Cleveland Clinic’s Lerner Research Institute and co-section head of Preventive Cardiology. “These findings underscore the importance of further long-term clinical studies to assess the cardiovascular safety of erythritol and other sugar substitutes.”

Smart knee technology allows care team to remotely monitor patients’ recovery

Technology allows care team to remotely monitor patients’ recovery

August 8 2024- MANKATO, Minn. — For more than 15 years, Ronald Gary Woods ended most days with aching knees.

“It was hard walking around. My knees would get sore and were popping and cracking,” says Woods, 63, of Eagle Lake, Minnesota. “It’s like everything else, they just wore out.”

Woods tried various treatments, including cortisone shots and nerve ablation, to interrupt pain signals sent to the brain. His plan was to wait until retirement for surgical intervention. “But I was tired of hurting,” he says.

Woods and Jacob Ziegler, M.D., his orthopedic surgeon at Mayo Clinic Health System in Mankato, recommended bilateral knee replacement, a surgery to replace both of his knees at the same time. Dr. Ziegler had recently become the first surgeon in the health system to begin utilizing new sensor technology licensed by Zimmer Biomet through a partnership with Canary Medical. The smart orthopedic implants function just like standard knee replacements, but they include a sensor in the stem anchored in the shin bone that remotely monitors and tracks a patient’s recovery by measuring range of motion, steps, stride and other data associated with gait.

Dr. Ziegler explained to Woods that he would have use of an app to see his gait and mobility information as he recovered from surgery. The data would be uploaded once daily to Dr. Ziegler’s team, helping them monitor how well Woods was progressing.

UPDATE FROM THE FLORIDA MEDICAL ASSOCIATION AUGUST 8 2024

Update on looming Medicare payment cut

By Jarrod Fowler, MHA, FMA Director of Health Care Policy and Innovation

What’s happening
Physicians face a 2.8-percent Medicare conversion factor cut next year. This significant pay reduction is largely the result of Congress’ unwillingness to statutorily fix the Medicare Physician Fee Schedule. For years now, physicians have been telling Congress that the Medicare Physician Fee Schedule is simply unsustainable. Years of cuts have further destabilized the traditional Medicare program and left physicians wondering whether they’ll have to cut back on providing care to vulnerable seniors to keep their practices economically solvent.

What is value-based care, and is it right for your organization?

Special to the FMA from Aledade

Value-based care is a healthcare concept that primary care organizations should be reimbursed for improved patient health outcomes, which differs from traditional fee-for-service models where doctors and healthcare providers are paid based upon services rendered.

Americans pay more for healthcare in comparison to those in other countries, with recent studies  indicating that health outcomes are not any better despite this expenditure.

In a value-based care arrangement, the primary goal is to keep patients well by meeting quality measures and specific performance metrics within value-based care contracts with Medicare, Medicaid and commercial health plans, in addition to the Medicare Shared Savings Program (MSSP). As a result, primary care organizations can focus on keeping their patients healthy while getting rewarded for it.

Breaking Language Barriers: HHS Issues Division-Specific Language Access Plans from Across the Department

August 8, 2024 – In November 2023, the U.S. Department of Health and Human Services (HHS) joined agencies across the federal government in prioritizing communication in its services to the public by releasing its department-wide Language Access Plan. With the release of its plan, HHS took a giant step towards ensuring people with limited English proficiency (LEP) and people with disabilities have greater access to the life-saving services that it provides.

Today, as a culmination of this effort, and in advance of the 24th anniversary of Executive Order 13166, “Improving Access to Services for Persons with Limited English Proficiency,” the Department has released division-specific language access plans. This effort by nearly all HHS operating and staff divisions exemplifies the Department’s commitment to providing accessible, culturally and linguistically competent, and comprehensive health care and human services.

“Removing the barriers that prevent people from accessing health care and human services support is one of our highest priorities,” said HHS Secretary Xavier Becerra.
